Treatment of patients with IgA nephropathy: a call for a new paradigm
Current treatments for IgA nephropathy (IgAN), including supportive care and corticosteroids, have limited success and significant side effects.
Advances in understanding IgAN’s pathogenesis have led to new targeted therapies and a proposed dual approach: targeting IgAN-specific mechanisms (e.g., pathogenic IgA production) and managing kidney disease progression with protective therapies like RAS and SGLT2 inhibitors.
Future treatment strategies will likely focus on integrating multiple therapies to maximize nephron preservation.

This study examined nephrologists’ perspectives on conservative kidney management (CKM) in the Middle East and North Africa region.
Results from the online survey showed that despite high awareness, only 20% of participants consistently offered CKM, and just 16% had a formal CKM program at their institution. The main barriers were financial constraints, resource shortages, and cultural factors.
Regional research is needed to address these challenges and improve CKM implementation.
