Measuring Albuminuria or Proteinuria: Does One Answer Fit All? 

The authors review international guideline recommendations on measuring proteinuria and albuminuria and discuss contexts where the optimal measure of proteinuria is unclear or controversial.

They discuss evidence to support current recommendations for choice of measure, including the clinical settings of glomerulonephritis, transplantation, and pregnancy. They also discuss how patient gender and economic considerations may impact this choice.

The urine albumin-to-creatinine ratio (ACR) appears appropriate in most settings. However, there is a lack of strong evidence that ACR is superior to the urine protein-to-creatinine ratio.

Targeting the Endothelin-A Receptor in IgA Nephropathy

This narrative review by Donald Kohan et al. discusses the use of endothelin-A receptor blockade and the potential role of this class of agents among the current and emerging therapies for treating immunoglobulin A nephropathy.

While these emerging therapies show promise, ongoing trials are still underway, and the most effective combination of therapeutic approaches has yet to be determined. Furthermore, to align with personalized medicine principles, optimal therapeutic strategies will need to be tailored for individual patients.
